As others have mentioned, it seems most common to split the syllables of  
the RTL script, and put them in their ordinary RTL direction under the  
relevant notes.
However, a while back Thomas Milo forwarded an interesting, and apparently  
rare, example of an early 20th century score which prints the musical  
notation in a right-to-left manner with reversed clefs, augmentation dots  
on the left, and so forth. It was *not* precisely a mirror image.
