No, it is not a particular design!
It is an error, namely confusion of U+201C with U+201F.

U+2018 and U+201C are *rotational* images of U+2019 and U+201D.
U+201B and U+201F are *mirrored* images of U+2019 and U+201D.
A few typefaces such as Courier New, Tahoma, Verdana have glyphs
in U+2018 and U+201C which really belong into U+201B and U+201F.

Interestingly, Microsoft has indeed corrected one typeface:
Trebuchet MS. The version supplied with Windows 2000 has wrong
quotation marks at U+2018 and U+201C. The latest version is right.
If you have access to both Windows 2000 and Windows XP/2003,
you should be able to see the difference.
Comic Sans MS, Courier New, Tahoma, Verdana are still waiting for
correction.
