attached is a picture showing another traditional method
of making the links between the text and the note:
the word commented upon is repeated in brackets.
For you -- because of file size not for the list --
a add a picture from the same qur'ân printed in Delhi
in 1306 h. showing that for a small number of notes per
page the footnote-marker-with-raised-digit is used.
So, I can reassure you that in real life, South Asian
Muslims did not use two digit above footnote markers;
they used three different footnote marker with each
up to 9 digits. When there are more than 27 notes
appear on a page, the (xx) method is used.
