As most of the entries came to me by email, mostly I did cut and paste.
Most came in native code pages and
I did conversions to utf-8. Some came as NCRs and a few (eg Braille) I
typed in as NCRs. Then I cut and paste the displayed results into a
program that gave me the utf-8.

(That may sound easy. You wouldn't believe how much work creating that
page was...)
